Vitamin D Status and Its Relation with Pediatric Non-Alcoholic Fatty Liver Disease

چکیده :
Background and Aim: Non-alcoholic fatty liver disease (NAFLD)-the most common chronic liver disease in children- is the hepatic manifestation of metabolic disorders with an increasing prevalence even in preschool-aged individuals. Vitamin D plays a pivotal role in the molecular pathways related to multiple disease, while its exact association with the severity of NAFLD in pediatric population remains unclear. This study aims to investigate the relationship between vitamin D status and the histological severity of NAFLD in children. Methods: We performed a comprehensive literature search in the online databases including PubMed, Scopus, Web of Science, and Google Scholar. In the search, we aimed to identify all studies examining the relation between vitamin D status and the severity of NAFLD in children by using a combination of MeSH (medical subject heading terms) and non-MeSH terms. Results: It is suggested that Vitamin D deficiency / insufficiency are prevalent among children with NAFLD. Meanwhile, the relationship between serum 25(OH) vitamin D levels and histological findings related to NAFLD remain contraversial. Reduced hepatic expression of vitamin D receptor is inversely associated with hepatic steatosis and lobular inflammation/ballooning. Additionally, vitamin D exhibits anti-inflammatory and anti-fibrotic properties that affects the progression of hepatic and extra-hepatic symptoms. Genetic polymorphisms in vitamin D metabolic pathways are linked to the severity of pediatric NAFLD, suggesting a potential role in disease susceptibility. Indeed, most recent cilinical trials have reported significant improvements in aminotransferases, lipid profile and liver biopsy, subsequent to vitamin D supplementation in children with NAFLD. Conclusion: Children with NAFLD frequently exhibit low vitamin D levels and recent findings highlight the favorable role of vitamin D supplementation, while the exact role of vitamin D in the progression of the disease remains unclear. Hence, conducting further well-designed studies with larger sample sizes are recommended.
