Relations of total physical activity and intensity to fitness and fatness in children

Background and Aim: The relationship between physical activity (PA)—specifically its total volume and varying intensities—and key health indicators such as cardiovascular fitness (CVF) and adiposity in children remains inadequately understood. This systematic review aimed to evaluate existing evidence on the associations between total PA, as well as moderate and vigorous PA intensities, with CVF and body fatness in children. Methods: A comprehensive search was conducted across databases (e.g., MEDLINE, EMBASE, Google Scholar, SCOPUS, Science Direct) covering studies published between 2019 and 2024. A total of 297 articles were initially retrieved. Studies examining children aged 9–10 years, were included. PA levels in this study, were objectively quantified using accelerometry and categorized into total, moderate, and vigorous intensities, expressed in minutes per day. CVF was assessed via maximal exercise testing on an ergometer cycle and reported relative to body mass (W/kg). Finally, 5 high-quality studies were selected for detailed analysis. Results: Evidence consistently indicated that higher levels of vigorous-intensity PA were significantly associated with lower body fat, whereas moderate or total PA did not demonstrate a similar relationship. Children accumulating more than 40 minutes of vigorous PA per day exhibited substantially lower fatness compared to peers with 10–18 minutes of such activity. In contrast, all levels of PA intensity—including total, moderate, and vigorous—were positively associated with improvements in CVF. Notably, children exceeding 40 minutes of vigorous PA daily showed enhanced CVF relative to those engaging in less than 18 minutes. Conclusion: The findings suggest a potentially superior role of vigorous-intensity PA in reducing adiposity among children, while both total and moderate-to-vigorous PA appear beneficial for enhancing cardiovascular fitness. These insights underscore the importance of PA intensity in pediatric health interventions.
