Effect of Probiotics on Weight and Body Mass Index in Children and Adolescents: A Meta-Analysis

Background and Aim: The gut microbiota has emerged as a key modulator of energy metabolism and weight regulation. Probiotic supplementation has been proposed as a potential therapeutic strategy for managing obesity in children and adolescents. However, evidence regarding its effectiveness remains inconsistent. This study aimed to quantitatively synthesize the available randomized controlled trials (RCTs) on the effects of probiotics on weight and body mass index (BMI) in the pediatric population. Methods: We systematically searched PubMed, Scopus, Web of Science, Cochrane Library, and Google Scholar up to March 2025. RCTs evaluating the effects of oral probiotic supplementation on weight and/or BMI in children and adolescents (≤18 years) were included. Two reviewers independently screened studies, extracted data, and assessed methodological quality using the Cochrane Risk of Bias 2 (RoB 2) tool. A random-effects model was applied to pool the data, and heterogeneity was assessed using the I² statistic. Results: A total of 14 RCTs comprising 1,132 participants met the inclusion criteria. Meta-analysis showed a modest but statistically significant reduction in BMI in the probiotic group compared to control (pooled SMD = –0.22; 95% CI: –0.39 to –0.05; p = 0.01). The effect on body weight was not statistically significant (MD = –0.84 kg; 95% CI: –2.02 to 0.34; p = 0.16). Subgroup analyses suggested that multi-strain probiotics and interventions lasting longer than 8 weeks had greater effects. Heterogeneity was moderate (I² = 51%). Conclusion: Probiotic supplementation may lead to a modest reduction in BMI among children and adolescents, particularly when multi-strain formulas and longer intervention durations are used. Further high-quality RCTs with larger sample sizes and standardized probiotic strains are needed to confirm these findings and clarify the mechanisms involved.
