A review of management and treatment of Inflammatory Bowel Diseases in children: challenges, novel approaches, and future research directions

چکیده :
Introduction: Inflammatory bowel diseases (IBD), like Crohn's disease and ulcerative colitis, are chronic gastrointestinal disorders that also affect children. Diagnosis is challenging because the symptoms overlap with other gastrointestinal diseases, requiring clinical, laboratory, and imaging examination. Methods: In this narrative review, an extensive search was conducted in the PubMed database and Google Scholar search engine using English keywords including IBD, crohn, ulcerative colitis, children, pediatrics.145 articles were found. After removing duplicates, outdated studies, and irrelevant content, 6 articles published between 2020 and 2025 were selected for analysis and review. Result : In inflammatory bowel disease (IBD), environmental factors play a more significant role than genetics. The higher prevalence of IBD among immigrants to countries with a high incidence suggests the impact of diet and microbiome changes. In pediatrics, not just clinical remission but mucosal healing as well as prevention of surgery and hospitalization is a goal of treatment. Due to challenges with endoscopy, tools like PUCAI and PCDAI are used for disease assessment. Treatment starts with low-risk medications and moves to more powerful ones if required. Drugs like 5-ASA and corticosteroids are used for induction, while biologics are applied for more severe cases. Improvements in treatment include early use of biologics and more precise monitoring, with individualized approaches aiming to improve long-term outcomes. Conclusion: Pediatric IBD treatment faces challenges due to the disease's complexity. An important limitation is that treatment is based on adult data in pediatric patients. Pediatric-specific trials and better biomarkers are necessary in future studies for individualized treatment.
