Prevalence of Autoimmune Thyroid Diseases in Children with Celiac Disease: A Study in Urmia

Background and Aim: Celiac disease is an autoimmune disorder affecting the small intestine due to gluten consumption. Besides the complications arising from digestive system involvement, many autoimmune issues, particularly autoimmune thyroid diseases, are also prevalent among these patients. This study aimed to investigate the prevalence of autoimmune thyroid diseases in children with celiac disease in Urmia to optimize their treatment management. Methods: In this cross-sectional study, venous blood samples were collected from children living in Urmia with previously confirmed celiac disease. The anti-transglutaminase test (anti-tTG) was conducted to confirm the diagnosis, while Antithyroid Peroxidase antibody (anti-TPO) and Antithyroglobulin Antibody (anti-TG) tests were performed to screen for autoimmune thyroid disease. Additionally, a Thyroid Stimulating Hormone (TSH) test was carried out to assess thyroid function, all using the ELISA method. Data were analyzed using descriptive statistics and the chi-square test. Results: Seventy-seven patients with an average age of 11.84 participated in the study; 30 were male and 47 were female. The Anti-TPO test was positive in 3.9% of the patients, while the Anti-TG test was positive in 11.7%, all of whom were female. The difference between genders regarding Anti-TG was statistically significant (p=0.023). In most patients with a positive Anti-TPO, the Anti-TG test was also positive, showing a significant positive correlation between these two tests (R=0.82). Hypothyroidism was observed in 26% of the patients, and hyperthyroidism was found in 1.3%. Among celiac patients with a positive Anti-TPO, hypothyroidism was present in 66.7%, and in those with a positive Anti-TG, hypothyroidism was seen in 45.5%, which was not statistically significant (p=0.12). Conclusion: The findings of this study indicated that thyroid autoimmune diseases are prevalent among celiac children, particularly in females. Consequently, regular assessment of thyroid function and monitoring of autoantibodies in celiac patients appears essential for the effective management of both conditions.
