Beyond Hunger: The Impact of Hedonic Eating on Pediatric Obesity

چکیده :
Background and Aim: Obesity constitutes a major global health concern with a prevalence escalating worldwide, particularly among children. This mulit-metabolic condition poses a significant challenge in pediatric population due to the early onset, limited success of conventional treatments, and its potential to precipitate chronic disease. Appetite regulation, a pivotal aspect of obesity, involves complex mechanisms that are not fully understood, especially regarding hedonic eating patterm. This study aimed to clarify the mechanisms of hunger and satiety in pediatric obesity with a view-point on hedonic eating,. Methods: Relevant articles were reviewed in online databases including PubMed, Scopus, Web of Science, and Google Scholar using a combination of MeSH (medical subject heading terms) and non-MeSH terms, and data were compiled from a selection of studies on pediatric obesity and appetite regulation. The review encompassed longitudinal and cross-sectional studies, focusing on hedonic hunger assessed via the Power of Food Scale (PFS), eating in the absence of hunger (EAH), and binge eating behaviors, alongside correlations with body mass index (BMI). Emphasis was placed on studies exploring neurological pathways, particularly the reward system. Results: Appetite is regulated through homeostatic mechanisms that stabilize energy balance, a weight set-point, and non-homeostatic mechanisms, including hedonic regulation, which promote eating for pleasure. Hedonic eating, characterized as consumption driven by reward rather than caloric need, is mediated by the brain’s reward circuitry. In children, higher preschool BMI predicted increased food responsiveness and diminished satiety sensitivity later in life. Children with overweight exhibited elevated EAH, with parental food restriction linked to increased snack intake in girls. Notably, the association between hedonic hunger and BMI appeared to be influenced by binge eating behaviors, highlighting the complexity of reward-driven eating in childhood obesity. Conclusion: Hedonic eating significantly contributes to childhood obesity via reward-driven behavior, underscoring the necessity for tailored interventions targeting these pathways
