Association between Plant-Based Dietary Index and Risk of Overweight and Obesity in Children and Adolescents

Abstract Background and Aim: Childhood and adolescent obesity remain major public health concerns globally. While plant-based dietary patterns are increasingly promoted for their health benefits, their association with obesity risk in youth is not well established. This study examined the relationship between plant-based dietary indices and overweight/obesity among U.S. children and adolescents. Methods: We used cross-sectional data from the National Health and Nutrition Examination Survey (NHANES) 2017–2020, including 1,652 participants aged 6–19 years. Three plant-based dietary indices were calculated from two 24-hour dietary recalls: the overall Plant-Based Diet Index (PDI), the Healthy PDI (hPDI), and the Unhealthy PDI (uPDI). Body mass index (BMI)-for-age percentiles were used to define overweight and obesity (≥85th percentile). Multivariable logistic regression models assessed associations between dietary index quartiles and the risk of overweight/obesity and central obesity (waist-to-height ratio ≥0.5), adjusting for age, sex, race/ethnicity, physical activity, and total energy intake. Results: Higher adherence to hPDI was associated with lower odds of overweight and obesity (Q4 vs. Q1: OR = 0.78; 95% CI: 0.66–0.93), while higher uPDI was positively associated with both overweight/obesity (OR = 1.41; 95% CI: 1.20–1.66) and central obesity (OR = 1.49; 95% CI: 1.25–1.78). These associations were stronger in adolescents compared to children and remained significant after full adjustment. No significant associations were observed for the overall PDI. Conclusion: Our findings suggest that a higher intake of unhealthy plant-based foods is linked to increased obesity risk, particularly in adolescents, whereas healthy plant-based diets may offer protective effects. Promoting the quality—not just the quantity—of plant-based foods may be critical in strategies to prevent childhood and adolescent obesity.
