The Effect of Gluten-Free Diet Adherence on Sleep Disorders in Children with Celiac: A Systematic Review

Background and Aim: Celiac disease is a chronic disease of the small intestine with a global prevalence of about 1%. Sleep disorders are common problems in these patients. Our search did not find a comprehensive review in this area, so this systematic review aimed to investigate the effect of a gluten-free diet on sleep disorders in children with celiac disease. Methods: In this systematic review, keywords related to 'gluten-free diet', 'sleep', 'children', and 'celiac disease' were searched in international and national databases Web of Science Core Collection, PubMed/Medline, Scopus, Irandoc, SID, Magiran and search engine Google_Scholar. The primary search yielded 15 studies. Inclusion criteria were publication in Persian/English. Grey literature came out. The time limit was not applied. After eliminating duplicates and critiquing with relevant tools, three studies were finally analyzed. Ethical considerations were observed and the abstract was reported according to PRISMA. Results: Studies have shown that a gluten-free diet significantly improves sleep quality in children with celiac disease in all subscales, regardless of gender, age, and symptom status and severity (p<0.001). This diet reduced sleep resistance, delayed sleep onset, and the frequency of sleep interruptions due to pain, bloating, or vomiting, thus increasing the duration and quality of nighttime sleepiness, leading to less daytime sleepiness. A gluten-free diet was able to improve obstructive sleep apnea. This improvement was observed in the snoring, sleepiness, and attention/behavior subscales. Also, adherence to this diet for 6 months led to a significant improvement in EEG abnormalities and headaches in these children and improved sleep quality. Conclusion: A gluten-free diet appears to have a positive effect on the sleep quality of children with celiac disease. It is suggested that future studies analyze the effect of psychological factors on the quality of sleep in these children.
