Association Between Sugar-Sweetened Beverage Consumption and Insulin Resistance in Obese Children: Focus on the HOMA-IR Index

Abstract Background and Aim Childhood obesity is a growing global public health concern, often associated with metabolic disorders such as insulin resistance, a precursor to type 2 diabetes. Sugar-sweetened beverages (SSBs) constitute a major source of added sugars in children’s diets and have been implicated in the pathogenesis of insulin resistance. This study aimed to investigate the association between SSB consumption and insulin resistance as measured by the Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) in obese children. Materials and Methods In this cross-sectional studys, dietary intake data were collected from a cohort of obese children aged 8–14 years using validated food frequency questionnaires. Fasting blood samples were analyzed for glucose and insulin levels, and HOMA-IR was calculated using the formula: (Fasting Insulin [µU/mL] × Fasting Glucose [mg/dL]) / 405. Multivariate linear regression models were used to assess the association between SSB intake and HOMA-IR, adjusting for age, sex, BMI, and physical activity level. Results Higher consumption of SSBs was significantly associated with elevated HOMA-IR values (p < 0.01), indicating greater insulin resistance. Children in the highest tertile of SSB intake exhibited a mean HOMA-IR that was 1.8 times higher than those in the lowest tertile. The association remained robust after adjusting for potential confounders. Conclusions The findings suggest that frequent consumption of sugar-sweetened beverages is independently associated with increased insulin resistance in obese children. Public health interventions targeting the reduction of SSB intake may play a critical role in mitigating early-onset metabolic dysfunction in pediatric populations.
