The impact of probiotic bacteria on healing and decreasing intestinal inflammation in children with cystic fibrosis

Background and Aim: Cystic fibrosis is characterized by intestinal inflammation. Probiotics may lower intestinal inflammation and the number of pulmonary exacerbations. We studied the makeup of the intestinal microbiota in children with cystic fibrosis and its association with intestinal inflammation. We also studied the microbiota structure before and after administration of Lactobacillus casei (LCG) in children with CF, both with and without antibiotic therapy. Methods: Electrophoresis, polymerase chain reaction (RT-PCR), and fluorescence in situ hybridization (FISH) were used to examine the intestinal flora. Intestinal inflammation was measured by measuring fecal calprotectin (CLP) and rectal nitric oxide (rNO) generation in children with CF compared with healthy controls. Results: The research included 40 children with cystic fibrosis. Children with cystic fibrosis had greater fecal CLP and rNO levels compared to healthy controls (121±173 μg/g vs. 26±37 μg/g; 11±11 vs. 1.2±1.1 μmol/L NO2-; P<0.01). Children with CF exhibited substantially altered gut microbial core compositions compared to healthy controls. A similar but more severe trend was found in children with cystic fibrosis using antibiotics. The use of LCG decreased fecal CLP while partly restoring gut flora. There was a strong link between lower microbial biodiversity and intestinal inflammation. Conclusion: CF causes quantitative and qualitative alterations in the gut flora, which may represent a new therapeutic target in the treatment of CF. Probiotic administration restored gut microbiota, indicating that probiotics are beneficial in lowering intestinal inflammation and pulmonary exacerbations.
