The Role of Probiotics in Preventing and Managing Gastrointestinal Disorders in Children: A Systematic Review

Abstract Background and Aim: The gut microbiota plays a major role in the development and function of pediatric gastrointestinal system, which an imbalance in gut microbial composition has been linked to various gastrointestinal disorders in children. The purpose of current study is to systematically evaluate the effect of probiotics in managing pediatric gastrointestinal disorders. Methods: A comprehensive search was performed on PubMed, Scopus, Web of Science, Cochrane Library until January 2025 to identify studies. All randomized controlled trials exploring probiotic interventions in children with gastrointestinal intestine disorders were included. Quality of included studies was assessed using Cochrane Collaboration’s tool. Results: Findings suggested that probiotics like Lactobacillus rhamnosus GG and Saccharomyces boulardii reduced diarrhea duration by ~1–2 days and Lactobacillus rhamnosus GG and S. boulardii reduced antibiotic-associated Diarrhea risk by ~50% in children. Stronger effects with early probiotic initiation alongside antibiotics. Multi-strain probiotics (e.g., Lactobacillus/Bifidobacterium blends) improve pain frequency and stool consistency in children with irritable bowel syndrome. Conclusion: In conclusion, probiotics intervention demonstrated strain-specific and disease-dependent efficacy in pediatric gastrointestinal disorders, with the strongest evidence for infectious diarrhea and antibiotic-associated diarrhea. Future research should focus on optimal strains and dosages for specific conditions.
