The Role of Nutrition in Managing Obesity in Children with ADHD: A Narrative Review

Background and Aim: Childhood obesity and Attention-Deficit/Hyperactivity Disorder (ADHD) frequently co-exist, sharing overlapping neurobiological, behavioral, and metabolic pathways. Poor dietary habits like high sugar intake, nutrient deficiencies, and consumption of processed foods contribute to dopamine dysregulation, impaired impulse control, and metabolic disturbances. This narrative review aimed to evaluate the role of nutrition and gut microbiome modulation as promising therapeutic strategies in managing ADHD and obesity in children. Methods: A narrative search was conducted across PubMed, Web of Science, Scopus, and Google Scholar databases up to 2025. After removing duplicates relevant RCTs, which evaluate the effects of dietary interventions, nutrient supplementation, microbiome-based therapies, or pharmacological-nutritional combinations in children with ADHD and obesity were screened. Data extraction and quality assessment were performed by the author based on the PRISMA guidelines. Results: Six RCTs were included. Two studies indicated that Low-Glycemic Index (Low-GI) diets significantly improved hyperactivity control (28%), reduced binge eating, stabilized blood glucose, and decreased BMI by 5.3%. The Mediterranean diet, rich in fiber, omega-3 fatty acids, and antioxidants, was associated with 7% lower obesity prevalence and reduced ADHD symptom severity. Two trials demonstrated that the Ketogenic diet led to a 14% improvement in cognitive flexibility, reduced sugar cravings, and a 5% reduction in BMI. One study evaluated GLP-1 receptor agonists (Liraglutide, Semaglutide) in ADHD-obese children, leading to a 12% reduction in BMI and improved emotional regulation through enhanced dopamine pathways. Additionally, one said probiotic supplementation increased dopamine precursor availability, improved gut health, and reduced binge eating behaviors by 18%. Conclusion: Nutritional strategies that include balanced dietary patterns, nutrient supplementation, and modulation of the gut microbiome, have shown beneficial effects on symptom reduction, weight management, metabolic regulation, and cognitive improvement in children with ADHD. Future personalized nutrition plans based on genetic and microbiome profiles are recommended. Keywords: ADHD; Childhood Obesity; Nutrition; Gut Microbiome.
