Effects of Probiotic Therapy on BMI, Waist Circumference, and Waist-to-Hip Ratio in Overweight and Obese Children: An Umbrella Review of Interventional Meta-Analyses

Abstract Background and Aim: Childhood obesity is a growing global health concern associated with multiple metabolic and cardiovascular risks. Probiotic therapy has been proposed as a potential intervention to regulate body weight and metabolic health. However, the overall evidence on its efficacy in reducing body mass index (BMI), waist circumference (WC), and waist-to-hip ratio (WHR) remains inconclusive. This umbrella review of interventional meta-analyses aims to synthesize the existing evidence on the effects of probiotic therapy on BMI, WC, and WHR in overweight and obese children. Methods: A systematic search was conducted in PubMed, Scopus, Web of Science, and Cochrane Library for meta-analyses of randomized controlled trials (RCTs) investigating the impact of probiotic supplementation on BMI, WC, and WHR in children with overweight or obesity until March 2025. Data extraction and quality assessment were performed using the AMSTAR-2 tool. Effect sizes, heterogeneity, and publication bias were analyzed. Results: Five meta-analyses including 1,656 participants were reviewed. The findings showed inconsistent effects of probiotic supplementation on BMI. Some studies reported significant reductions, while others observed increases or no effect. For WC, three studies showed a significant decrease across all studies. WHR was not significantly affected by probiotic supplementation. Variations in results may be due to differences in probiotic strains, dosages, intervention durations, and participant characteristics. Conclusion: Probiotic supplementation may have a beneficial but limited effect on BMI in overweight and obese children, while it has been shown to significantly reduce WC. Further high-quality RCTs with standardized protocols are needed to confirm these findings and determine optimal probiotic formulations for weight management in pediatric populations.
