The association of gut microbiota in obese children with non-alcoholic fatty liver disease: A Review

چکیده :
Background and Aim: Non-alcoholic fatty liver disease (NAFLD) and obesity in children are pressing public health challenges, with their prevalence rising alarmingly around the globe. Growing evidence highlights the gut microbiota's crucial role in the onset and exacerbation of obesity-related NAFLD. This study aims to explore how gut microbiome composition and dysbiosis impact pediatric obesity and NAFLD, positioning this area as a vital target for innovative therapies. Methods: We conducted a review of literature published between 2021 and 2025, focusing on the interplay between gut microbiota and children with NAFLD. Keywords used for searching literature were: NAFLD, Obesity, Childhood, Gut, Microbiome, Pediatric. Results: Our findings reveal that gut dysbiosis—marked by a decline in beneficial bacterial diversity and an elevated ratio of Firmicutes to Bacteroidetes—is closely linked to pediatric obesity and NAFLD. Alterations in microbial metabolites, including short-chain fatty acids and bile acids, significantly influence energy metabolism and fat accumulation in the liver. Furthermore, heightened intestinal permeability and the presence of microbial endotoxins drive systemic inflammation and contribute to liver fat buildup. Conclusion: The gut microbiome plays a pivotal role in the development of obesity and NAFLD in children. Targeting the microbiome through strategic dietary changes, probiotics, and other interventions presents an exciting opportunity to effectively manage these conditions. It is essential to conduct further research to pinpoint specific microbial signatures linked to disease severity and to develop tailored therapeutic strategies.
