The Association between VITAMIN E INTAKE and Non-Alcoholic Fatty Liver Disease : An Umbrella Systematic Review , Meta-analysis.

Abstract Background and Aim: Non-alcoholic fatty liver disease (NAFLD) is a growing global health concern, with oxidative stress playing a key role in its progression. Vitamin E, a potent antioxidant, has been proposed as a potential therapeutic agent. This meta-analysis evaluates the efficacy of vitamin E supplementation on liver enzymes (ALT, AST) and fibrosis levels in NAFLD patients, with a focus on dosage and intervention duration. Methods: A systematic search was conducted across multiple databases to identify randomized controlled trials (RCTs) assessing the effects of vitamin E on NAFLD. Six studies (n=2306 participants) were included for ALT/AST analysis, and five studies for fibrosis assessment. Pooled mean differences (MD) with 95% confidence intervals (CI) were calculated using a random-effects model. Subgroup analyses were performed based on dosage (>500 IU/day) and intervention duration (>20 months). Heterogeneity was assessed using I² statistics. Results: - ALT levels: Vitamin E significantly reduced ALT (MD= -6.47 U/L, 95% CI: -11.73 to -1.22, I²=58.2%). Greater reductions were observed with doses >500 IU/day (MD= -6.69 U/L, -11.53 to -1.74) and interventions >20 months (MD= -6.74 U/L, -9.26 to -4.23). - AST levels: A significant decrease in AST was found (MD= -5.35 U/L, -9.78 to -0.92, I²=42.9%), with stronger effects at >500 IU/day (MD= -6.44 U/L, -9.85 to -3.02) and >20 months (MD= -6.93 U/L, -9.41 to -2.45). - Liver fibrosis: Vitamin E modestly reduced fibrosis (MD= -0.24, -0.36 to -0.12, I²=0%). Conclusion: Vitamin E supplementation, particularly at doses >500 IU/day and durations >20 months, significantly improves liver enzymes and mildly attenuates fibrosis in NAFLD. These findings support its therapeutic potential, though further long-term studies are needed to confirm its impact on histopathological outcomes.
