Evaluation of Insulin Resistance indexes in Children with Non-Alcoholic Fatty Liver Disease (NAFLD)

Abstract Background and Aim: Non-alcoholic fatty liver disease (NAFLD) is becoming an increasingly serious health issue among children, particularly due to its strong ties to obesity and metabolic disorders. Insulin resistance (IR) plays a crucial role in the progression of NAFLD, yet we still have a lot to learn about the specific indicators of IR and what they mean for kids with this condition. This narrative review sets out to explore how IR indices relate to NAFLD in children, emphasizing their importance for diagnosis and prognosis. Methods: We conducted a thorough review of the existing literature, focusing on studies that looked at IR indices (like HOMA-IR, QUICKI, and the Matsuda Index) in pediatric NAFLD. We gathered and compared data to see how well these indices predict the severity of the disease, its progression, and how children respond to various treatments. Results: We found that higher levels of HOMA-IR were consistently linked to more severe cases of NAFLD in children, while QUICKI and the Matsuda Index showed an inverse relationship. Some research indicated that dynamic IR indices (such as postprandial measurements) might provide a clearer picture of metabolic dysfunction compared to fasting markers. Moreover, these IR indices were associated with histopathological findings, including the degree of fibrosis and steatosis, although their accuracy as standalone diagnostic tools varied. Conclusion: IR indices, especially HOMA-IR, are important for evaluating metabolic dysfunction in pediatric NAFLD. However, their limitations point to the need for additional biomarkers or imaging methods. Future studies should aim to establish standardized cut-off values and investigate the long-term relationships between IR indices and clinical outcomes.
