چکیده :
Background and Aim: Childhood obesity is a global epidemic linked to metabolic complications, including insulin resistance (IR) and vitamin D (vit D) deficiency. While the association between obesity and IR is established, the role of vit D in modulating IR among obese pediatric patients remains underexplored. This study investigates the relationship between vit D deficiency and IR in obese children, proposing targeted interventions to mitigate metabolic risks. Methods: A cross-sectional study of 150 obese children (BMI ≥95th percentile, aged 6–18 years) was conducted. Serum 25-hydroxyvitamin D (25(OH)D), fasting glucose, insulin, and HOMA-IR were measured. Demographic, anthropometric, and lifestyle data were collected. Statistical analyses included multivariate regression and subgroup stratification. Results: Vit D deficiency (25(OH)D <20 ng/mL) was prevalent in 58% of participants. HOMA-IR ≥2.5 was observed in 47%. Inverse correlations between 25(OH)D and HOMA-IR (r = -0.42, p <0.001) and BMI z-score (r = -0.38, p = 0.002) were identified. Severe obesity (BMI z-score >3) amplified IR risk (OR = 3.1, 95% CI: 1.8–5.4). Sedentary behavior (>4 hours/day screen time) independently predicted lower 25(OH)D levels (β = -6.2, p = 0.01). Conclusion: Vit D deficiency exacerbates IR in obese children, particularly in severe obesity. Integrative interventions combining high-dose vit D supplementation and lifestyle modification are proposed to address this metabolic synergy.
